c.   Site Survey - from Registered LA PLS including existing residential land use on adjacent propert and existing towers within 5 square miles.    
d.   Vicinity map showing location of applicants's antenna and all existing towers within 3 square miles of the proposed site.    
e.   Certificate of tower structure capability and conformance with EIA/TIA-222    
f.   Identification of all property, equipment and antennas to be located on the site and their owners    
g.   Written authorization from the site owner allowing the applicant to submit the permit    
h.   Certification by applicant the tower and installation will meet FAA requirements
